Does this combo actually work?

They way I am reading it, If a player casts a spell they either have to discard or loose 5 life, and the discard causes the Quest to get counters as well as hold them down on cards because if they drop to zero cards in hand they will loose 5 life during their upkeep.

Double question! Same answer applies though. It could work in a dedicated discard deck, yes, but you're better off running card:Liliana's Caress or Bloodchief Ascension ; they're cheaper to get out, which means you can play them sooner, which means you can probably kill them by the time you'd cast a Painful Quandary and have your opponent fault prey to it.

Now if you were running a mono-black EDH with Liliana Vess in it to fetch one out, it's a bit more viable there...

I actually build a discard themed MBC deck that uses Painful Quandary and Liliana as possible backup win cons. I dont have Quest in it, but I have been considering making room for it. The only real problem with Quest is that it doesnt actually do anything by itself. Dropping one on turn 1 is great for a discard deck, but getting one late game will leave you with a dead draw more often than not. There is also the other requirement of your opponent not actually having cards in hand, which is why this card isnt a very popular choice.

Combine all those ideas with a card:Liliana's Specter on a Mimic Vat to keep them discarding and taking dmg? keeps the quest viable mid/late but is counter productive with Painful Quandary but its your call to, either discard that last card or lock their hand down to 1 card taking 2 each turn due to card:Liliana's Caress, either way they're screwed ;)
