RULES--this is a challenge NOT a contest there is no winner and the only prize is bragging rights
-this deck must be a 60 card MODERN legal deck
-this contest will never officially end
-your deck Must be designed to consistently kill yourself (yes making YOU lose) on turn 1
-have fun be creative and make a terrible deck

T1 Immolating Souleater . It also doubles as a window conditioner if you feel like winning.

You know, a "Worst Deck Ever" with this definition doesn't even seem like the worst deck ever. At least killing yourself Turn 1 is efficient and gifts your opponent victory quickly. Why not a deck that kills yourself really, really slowly? Or fail to do even that?

Very consistent... 50 fetchlands, 10 shocklands, fetch and play untapped, will win. Choose to go second!

turn one Pact of Negation or Slaughter Pact or Pact of the Titan or Summoner's Pact or Intervention Pact That is essentially dooming yourself on the first turn, and your opponent can't save you

Not to spoil anything, but I'd probably go with Plunge into Darkness and Spoils of the Vault to kill myself. Gitaxian Probe and Street Wraith to dig, Manamorphose to make sure you can cast spells, rituals to ramp mana, and suddenly you're playing Goblin Charbelcher targetting yourself.
