Your curve is very high and you need way more mana dorks/ramp. Low CMC options include: Elvish Mystic , Fyndhorn Elves , Boreal Druid , Birds of Paradise , Honored Hierarch , Orcish Lumberjack , Beastcaller Savant , Copper Myr , Drover of the Mighty , Druid of the Cowl , Golden Hind , Gyre Sage , Harvester Druid , Heart Warden , Hedron Crawler , Iron Myr , Leaf Gilder , Leafkin Druid , Sylvan Caryatid , and Lotus Cobra .
Might also benefit from being able to play creatures with flash: Shimmer Myr and Yeva, Nature's Herald
Prowling Serpopard would be good for making your creatures uncounterable.

The switches lists speed the deck up and make it win through an easy and efficient combo, all fetchable by Yisan. The combo uses Great Oak Guardian , Temur Sabertooth , and Wirewood Symbiote . With these and enough mana producing creatures, you get infinitely large creatures or with 1 more mana, infinitely large creatures and infinite mana, and untaps which can be put into Yisan to fetch Caterhoof and give the team trample.
All together, the deck will also run enough early-game interaction like Caustic Caterpillar , Manglehorn , Reclamation Sage , Natural State , Nature's Claim , the already run Lignify and Acidic Slime , and catch-alls like Desert Twister and Yisan fetchables like Somberwald Stag .
With all of this in mind, it is a little weak to removal, but with Eternal Witness and Temur Sabertooth both fetchable with Yisan, I feel it can be okay.

Hi! Looks like a good start to me! Here are some suggestions:
Additions:
- Priest of Titania. This card is seriously broken.
- Elvish Clancaller. While you won't be using the second ability, a 2-mana lord is always good.
- Joraga Treespeaker is an excellent mana dork.
- Wirewood Symbiote is also rather broken. Bounce your Elvish Visionary to untap your Priest of Titania, drawing more cards and making more mana? Yes please.
- Growing Rites of Itlimoc Flip. Gaea's Cradle is very expensive; this is significantly less so, but also very powerful.
- Wirewood Lodge. Opportunity cost is tiny, and can net you so much mana when it's useful.
- Nykthos, Shrine to Nyx. Same as above.
- Song of the Dryads. Mono-Green has problems dealing with creatures; this card would be basically unplayable in any other color, but is very good here.
- Craterhoof Behemoth just wins games.
- Triumph of the Hordes. Same as above.
- Regal Force can draw tons of cards.
- Krosan Grip is just powerful utility removal. Should be in every green deck IMO.
- Door of Destinies makes your 1/1s into serious threats.
- Heroic Intervention can protect your board from targeted removal and board wipes alike.
Cuts:
- Prism Ring is just bad. You don't have any lifegain synergies, and it's just not worth the card.
- Blunt the Assault is also just plain bad. I don't think you need Fog effects, but if you do, simple Fog is probably better.
- Nevinyrral's Disk. You have a lot of creatures, and usually don't want to be wiping the board.
- Krosan Restorer. Good mana dork, but not an elf.
- Karametra's Acolyte. Same.
- Llanowar Mentor. This card is cute, but not that powerful in practice.
- Might of the Masses. Combat tricks are usually pretty bad in Commander, and this one is no exception.
- Wirewood Pride. Same.
- Presence of Gond. Unless you're going infinite (which you're not), this card just doesn't do much.
- Elvish Pioneer is a bad mana dork.
- Leaf Gilder. Same as above.
- Elvish Soultiller. It would be one thing if this added them to your hand, but just shuffling cards into your library isn't that powerful.
- Viridian Zealot. There are better Naturalize effects.

Nice Deck, I also love my Trostania as a commander. Some suggestions:
You should use Llanowar Elves instead of Leaf Gilder.
Also instead of some expensive Sorcerys you can go for more consistent token generators like Crested Sunmare , Darien, King of Kjeldor (Darien is insane when Trostani is out, add a token doubler and gg), Master of the Wild Hunt, Sandwurm Convergence, Legion's Landing or Tendershoot Dryad. Kjeldoran Outpost could also work well.

Stazeeee, thanks for the upvote at Radiant Elves.
I get a lot of comments suggesting to add Heroic Intervention and it's fine if you want to play it, add it, but I don't. Intervention doesn't do enough in matchups where I want it, control matchups. Overextending my army because I'm counting or hoping that Intervention can save it is not a good plan. Instead my plan is to sideboard in cards that don't care about Fumigate or Settle because ultimately I can't stop all the Fumigates and Settles that my opponent will have. I want to sideboard in noncreature cards that can combat Teferi. Nissa, Vital Force's haste 5/5 lands and Lifecrafter's Bestiary to match card advantage; Bestiary's draw works through counterspells.
Due to tribal I only want to sideboard in a few cards in matchups; I can't take out very many cards especially Elves. I want to be aggressive in the control matchup, but there's a fine line between being aggressive and being dumb and overextending. Without having cards like Nissa and Bestiary at my disposal then my only realistic plan is to overextend with Intervention protection, which is problematic.
I like Song of Freyalise I think it's a great card, but not currently with Elves. Currently in Standard the best Elves other than Steel Leaf Champion produce mana. Elves don't have a problem with making mana therefore the first two chapter abilities of Song are not needed. The third chapter ability is amazing, but I don't feel the card is worth it because for two turns it's not doing much.
Many versions ago this deck was called "Rite Elves" utilizing Cryptolith Rite to ramp into a fast Westvale Abbey Flip or Distended Mindbender with Elves. It had Collected Company and Shaman of the Pack and other Elves. In the version I cut all mana Elves other than Leaf Gilder and this worked with Rite because of a large amount of powerful two drop nonmana Elves especially Dwynen's Elite. Gilder stayed because mana it produces could be used to cast a turn three Company without needing other Elves. In current Standard the two drop spot for Elves is vacant, with the least amount of options for the tribe. Song really needs a new Elite or a reprint of Elite or Elf token producers to be good with Elves. If there's enough good one or two drop nonmana Elves or Elves get a token theme post rotation then Song can be a possible fantastic addition.
Sandwurm Convergence is a good suggestion, but I think it's CMC is a little too high to be used to stop those flying threats you mentioned. If we had to stop high 6-8+ CMC Dragons, Demons or other big fliers then Convergence would be better. Relying on stopping four, five or less drop fliers with an eight mana card seems too slow. Making a 5/5 Wurm each turn is great if midrange was a problem right now in Standard then this could be good, but it's not a problem, aggro is and those fliers you mentioned are played with aggro. Convergence is not something I want in aggro matchups. You could try cutting Crushing Canopy for Convergence as it stops Lyra, but Canopy is also a way to stop History of Benalia in the same matchup where opponent is playing white and Lyra.

I will probably have to add a Manglehorn, that is a good and very relevant card. Thanks for the suggestion.
Lotus Cobra, however, will provide 1 mana per turn at best in my deck unless I cast and resolve a Primal Surge (at which point I won't need the mana), since I didn't put other ways to play multiple lands per turn into the deck. Thus it would be essentially serving as a less reliable Leaf Gilder, which just doesn't seem worthwhile, especially at $5 per copy and with how many 2 drops I already have.

The Channeler Initiate and Drover of the Mighty are in because this list is super super light on lands, which means that we need to make up for it in some of the nonland ramp. Tapping for multiple colors is more important here than the elf typing, though I might add in things like Wirewood Elf and Leaf Gilder sometime.
The Elvish Visionary change is actually somewhat recent, and I haven't updated it here yet. As it stands, since we have Heart Warden as our wincon of choice, I'd probably swap out Elvish Visionary for another ramp piece.
Lys Alana Huntmaster was the old Hackball wincon before we discovered Phantasmal Image, It's cheaper, and thus might be fine if you can't afford Phantasmal, but I'd greatly prefer Phantasmal Image due to its usefulness earlier in the combo turn. If you can't afford Phantasmal, a better option might be Altered Ego.
Finally, the 3 mana ramp pieces like Elvish Guidance and Elvish Archdruid aren't in because our 5 mana need can be met with 2 dorks and 3 lands. This means stuff like a 1cmc dork plus a 2cmc dork, plus 3 lands, would get us to 5 mana just as easily as a 1cmc dork and Marwyn, for instance. Thus, this list will stick to the cheaper forms of ramp, since they add up easier.
