SP3CTR3_chelts no need for discard effects when they’ll be forced to discard a couple cards during their cleanup step. That and Helm of the Ghastlord and or Elder Mastery attached to Nekusar will force them to discard every card they draw.

Full of fun effects and flavor, but you don't call things to my attention for flavor shenanigans. You call things to my attention for cold, ruthless efficiency.
First of all, you have cards that pull you in directions that the deck...just isn't going. Malfegor wants you to discard your hand, but you like having a full hand of tricks. Ashnod's Altar wants you to sacrifice creatures, but most of your creatures aren't expendable. God-Pharaoh's Gift wants you to reanimate things smaller than 4/4s, but most of the things you'd want to reanimate are bigger. Not sure you have enough cards (dragons and instants/sorceries) for Bladewing the Risen or Augur of Bolas. Fraying Omnipotence hurts you just as much or more as it does the other players, and Apex of Power is neat in theory, but with the number of big, expensive things you're running, you'll usually be casting only one card off of it. Gate to the Afterlife wants you to have things dying, but your things are too expensive to kill so haphazardly.
Then there's a few things that just suffer from the scale of EDH. Tormenting Voice can do work in mono-red, but you have blue. Play blue card draw. It's just...better. Even the cheap stuff. Hell, Chart a Course is a bad option and it's almost strictly better. Eternal of Harsh Truths doesn't do all that much here, just drawing you a few cards...whopee. It's EDH. Nobody cares. Likewise, Undermine has pretty much no upside over Cancel, and if you can, I'd try to find a better counterspell. For Elder Mastery, I'd be hesitant including it, because it bares your throat to removal or tempo plays for not...too much gain, because if you can leverage it to advantage, you're probably already winning. Sadly, except in niche cases, Wit's End isn't super effective in EDH because it won't hit for more than a few cards in almost all cases.
And never forget in EDH, your general matters. When you flip Bolas, you get a super powerful 'walker and an endless stream of cards. To fully take advantage of this, play removal, a few cheap counterspells, and most importantly, MORE RAMP. You are a very top-heavy deck. You gotta double down on that ramp; you'll always have something to do with your mana, especially with Bolas in the helm.
I have more optimizing comments, but those are the big glaring problems I see. Others are suboptimal things that you might want to keep for fun and trolling (I know how you play).
For card suggestions, peruse Lord Bolas. It is a rather expensive list, at times, but it can give you an idea of how to work with the Ravager as a commander.

Elder Mastery really isn't a good card to splash an entire color for. You'll never make people discad with it that late in the game.
Mindcrank is just not that good outside of very certain combo decks.
Burning Inquiry doesn't really do anything for you, since you don't have enything in the main that triggers upon discarding, it's a 0 for 1.
Elixir of Immortality also needs a very certain shell to be playable in the main.
I recommend getting Lightning Bolt it's an all 'round good card that can both serve as removal of creatures and planeswalkers, and as direct damage. For just .
I understand if the most played disrupts are too expensive, but if you run into any; Inquisition of Kozilek would be great.

I'm not a fan of mindcrank without Bloodchief Ascension. It almost never pays off, and so many decks want cards in their graveyard you can get punished pretty hard. Jamming Rakdos Charm maindeck can offset some of that, but giving your opponent card advantage is a real risk. Burning inquiry carries similar problems, and without you running lots of cards that want to be in your graveyard, will likely help your opponent more than you.
I'm guessing this started as a delayed damage Emberwilde Augur Insult Rift Bolt list, possibly including Flame of Keld, which then morphed into a Waste Not variant. Its a fun idea- I've never made it work without it becoming a different deck.
First things first, if you want a damage/discard hybrid deck, you need to decide- how important is the discard? Is the this the rack, or is it a burn deck with blightnings?
If your meta has aggro decks that typically only have 1 card in hand turn four, a lot of discard is wasted. If its all control, then you'll want to be jamming as much as you possibly can just to keep from being locked out of the game. The big thing is that with fifteen cards sideboard, you can pack in 11 copies of duress variants and 4 shrieking afflictions. The sideboard is there to adapt you to your meta- whether that be by jamming silver bullet answers or completely changing your strategy in response. Just by swapping Lightning Bolt alikes for Duress you completely change the pace and playstyle of your deck.
I have a big list of mono black cards I use for brews here Mono black deckbuilder's toolbox, in particular Sign in Blood and Night's Whisper- both net you more cards in a way that red struggles with, and in a pinch can finish off someone on five life with lighting bolt.
Elixir of immortality is cute. Personally, I prefer Sun Droplet or the new Fountain of Renewal, and sometimes Golden Urn. Shuffling the graveyard back in is usually a disadvantage outside of 'draw your deck' combos.
Finally, remember to build something that is fun to play. Its all well and good knowing the 'best' option, but if it ain't fun, it ain't worth it. So always keep that in mind when choosing cards.

There's a printing of Countersquall from the Ajani vs Bolas duel decks that has Bolas in the art and flavor text. Cruel Ultimatum is an absolutely massive spell, and you can get one with artwork with Nicol Bolas in it. Hour of Devastation is a board clear and extremely Bolas-flavored. Dragons of Tarkir's printing of Tormenting Voice has Bolas in it. Wit's End from M13 also has Bolas.
Blood Reckoning, Cower in Fear, Crook of Condemnation, Doomfall, Elder Mastery, Imp's Mischief, Magmaquake, Mastermind's Acquisition, Mindclaw Shaman, Neheb, the Eternal, Rakka Mar, and Soul Manipulation all have flavor text attributed to, or involving Nicol Bolas.

Personally, when it comes to nekusar, I have found infect isn't the way to go. What really works for me is stuff that prevents your opponents from playing stuff at instant speed combined with Nekusar, the Mindrazer enchanted with Helm of the Ghastlord, and enough draw engines to ensure you draw into a combo otherwise.
