9) Segovian Leviathan, but I'd like to point out that the flavor text has no reference to the book of Jonah, and "Leviathan" is not the name given to the great fish that swallowed Jonah.
14) Is it Olivia Voldaren? For some reason I'm having a lot of trouble finding a large image of the art, but I do remember some freckles.

Traditional pronunciations of "R'lyeh" are "Rihl-YEH" and "Ruh-LAY." Will confirm answers a bit later.
