This is a pretty standard Tribal list which either wins by ammassing as many Zombies and Lords as possible and grinding the table out or by comboing off.

While Gisa and Geralf is my default Commander, it easily be switch with The Scarab God or Grimgrin, Corpse-Born without losing much power. If you playing at a rather high power table, The Scarab God is probably the best option to lead your Horde, while Grimgrin, Corpse-Born is a lot of fun at a more casual table.

Now for the combos in this deck. Most of them revolve around Gravecrawler in some fashion:

Gravecrawler+Phyrexian Altar+Plague Belcher/Diregraf Captain= infinte drain

Gravecrawler+Grimgrin, Corpse-Born+Rooftop Storm= infinte big Grimgrin

Gravecrawler+Phyrexian Altar+Rooftop Storm+ any Zombie= infinte colored Mana

Gravecrawler+Ashnod's Altar+Rooftop Storm+ any Zombie= infinte colorless Mana

For the infinite Mana combos you could either use Exsanguinate, Geth, Lord of the Vault or Blue Sun's Zenith to close out the game.

Another combo that's not in this deck anymore is Mikaeus, the Unhallowed+Triskelion. I threw it out mostly because triskelion on it's own has no synergie with the rest of the deck and because people tend to REALLY dislike this combo.
